Intake Questions for New Clients

After a client moves through the on-boarding process and selects a coach from the Fitmo Marketplace (please see What Does On-Boarding Look like for Clients for additional info), there are a couple more steps a client is taken through:
  1.  Once the client selects a coach, the are taken to the messaging feed.
  2. A message that is automated by Fitmo, yet looks and sounds as though it was personally written by the coach, appears, welcoming the client and inviting them to share more info about themselves and their health goal by filling out the Intake Form.
  3. Inside this^ message, there is a direct link to the Intake Form, which contains a message and 5 questions automated by Fitmo. Please see the 'Intake Form and Questions' below to familiarize yourself with what questions all clients are asked.
  4. Once a client completes the Intake Form, their answers and personal information can be found by the coach inside the Client's Profile under the 'Profile' tab and then 'Intake' section. Coaches should reference this section to learn as much as possible about the client and move forward accordingly.
Please Note:
  • Some clients opt to not answer the Intake Form. As a coach, that's not a problem at all! The most important thing is to get the connection and conversation going by messaging the client right away and asking them questions in the messaging feed, allowing coach & client a great opportunity to get to know one another.
  • As Fitmo expands their coaching team to include coaches focused on more than just fitness, they will develop and implement a separate Intake Form for clients who've chosen a Nutrition or Wellness goal. Stay tuned!
